New Patient Forms
Because new patients require additional time with our Medical Assistants, and time setting up your patient chart, we ask that you print the new patient forms and return them to our office filled out in their entirety, prior to your initial appointment.
You may fax your completed forms to us back to us at 972-867-4766, prior to your scheduled appointment date, or you may log in to your patient portal and electronically fill out the necessary information. If you have not been given the username and password for your patient portal, please contact a member of our front office team at, 972-596-6400, and select Option 4. One of our team members will be happy to assist you with setting up your patient portal so that you may register online.
New patients not submitting completed forms, or completing their new patient registration online at least 24 hours prior to their initial appointment, will be called by one of our team members to reschedule the appointment.
NEW PATIENTS : Please plan on arriving 30 minutes prior to your scheduled appointment time, or your appointment may be rescheduled.
Please note that this list does not necessarily include every plan we accept. We often find we are "in network" with a smaller plan, simply because the plan is under the umbrella of a larger plan. As of January 1, 2020, although we are in-network with other Medicare Advantage plans, we will no longer be accepting any Medicare Advantage plans other than United Healthcare's AARP Secure Horizons, Plans 1, 2, and 3 that work with the WellMed care model.

Due to the additional time that is required to establish new patients, we request that you arrive 30 minutes prior to your scheduled appointment time. This allows our staff to ensure that all of your paperwork is complete, that your insurance information has been verified, and take your photo for your chart.
Please also note that new patient appointments are billed at a higher rate than subsequent visits. This is due to the additional time required, and documentation of allergies, medications, social history, surgical history, etc. If you are a self-pay patient, we do offer a 15% discount on your office visit, and 20% off of any lab tests, provided you pay in full at the time of service.
All co-pays, co-insurance and deductible amounts are due at the time of service. If payment arrangements are required, they must be made prior to your appointment date.
If you have scheduled an appointment as a new patient, and "no-show" for your first appointment, no further visits will be scheduled for you. Cancellations are accepted for new patients, 24 hours in advance of your appointment time.
We do NOT prescribe Schedule II pain relievers. Patients requiring treatment for chronic pain will be referred to a Pain Management Specialist.
We do NOT diagnose ADD or ADHD. Diagnosis must be made by a licensed psychiatrist. Once a diagnosis has been established, we will happily manage symptoms and medications. New patients with a diagnosis MUST provide health records from the diagnosing physician which contain the diagnosis, as well as any accompanying prescription history PRIOR to being seen in our office for their first appointment. If medical records have not been received by the time of the appointment, we will reschedule the visit once records have been made available. If you are a new patient who is being seen in our office for the first time, or if you have been prescribed, or wish to be prescribed, any controlled substance (such as Adderall, Ambien, etc), we may conduct a urine and/or oral toxicology screen.
We are a "participating" or "assigned" Medicare provider. This means that we file the claim to Medicare for you, and if you have a bridge to your supplemental plan established, Medicare will forward the claim to your supplemental plan once Medicare has processed the claim. If you have a supplemental plan, we will need that information in order to file claims for you.
Our office performs compliance monitoring of prescription medications as a part of our ongoing commitment to provide the highest quality healthcare for our patients.
